If you wish to register on line please click on the link below to open our registration form

Please print the form out and complete it. Please return the completed form to us.

We will then register you as a patient. When you attend for your first appointment you will be asked to sign the form to confirm that all your details are correct.

PLEASE NOTE: IT IS IMPORTANT THAT YOU FULLY COMPLETE EVERY SECTION OF THE REGISTRATION FORM. FAILURE TO DO SO WILL MEAN THAT WE ARE UNABLE TO ADD YOU TO OUR PATIENT LIST UNTIL YOU HAVE PROVIDED US WITH THE MISSING INFORMATION.

Practice Area

We currently accept patients from Wool, Bovington, East Burton, East Stoke, Coombe Keynes, East and West Lulworth, East Knighton, Winfrith Newburgh, East and West Chaldon and Owermoigne.

We will not be able to keep patients who move outside this area on our list. If you move, however nearby, please always notify us of your new address and phone number and check you can remain registered. (We do not accept registrations from Wareham; Crossways; Bere Regis; Tincleton, or Holworth and beyond.) 

If you are under the care of the hospital please remember to let them know your change of address.
